Root cause Connection pool saturation on the webhook delivery worker following a deploy that doubled the per-workspace connection limit.

Resolution Rolled back the deploy at 09:52 UTC. Queue drained by 10:44 UTC. Connection pool sizing now derived from active-workspace count rather than a static value. Public post-mortem published 2026-04-23.

Impact Reconciliation reports lagged 4–6 minutes behind real-time. Console balances accurate; reconciliation feed delayed.

Root cause A single TON RPC provider returned stale block headers for 11 minutes. Indexer's multi-source verification correctly held back — by design.

Resolution Provider self-recovered. Multi-source policy unchanged. No client action required.
